Mass Youth Soccer CORI Form
All coaches and assistant coaches are required to complete the CORI form which is used to conduct coach background checks.

Nashoba Coach’s Pledge Form on our website for easy downloading by coaches and assistant coaches. This is a required form to be filled out by at least one coaching member of each Nashoba team.

Administration Player Evaluation spreadsheet HYSA Player Evaluation
A few weeks before the end of the fall season return a player evaluation form for our use in the team formation process for next season. Please take the time to carefully evaluate your players relative to players you’ve seen on opposing teams and similar Hudson teams. I’ve copied some of the assistant coaches to allow them to develop their own scoring, to compare with your own evaluation, but I ask that only the head coach submit the final evaluation to the president.
In the attached form, please fill in the highlighted yellow areas and a summary scoring sheet will develop from the information or scores you input on the second worksheet. Please feel free to score each individual player using decimals such as 3.20, rather than 2 or 3, to allow you to truly differentiate the skills you see with your players………only one player can be your best offensive weapon, your best sweeper, your best keeper……..I’m making you think hard about each player’s skills to develop an objective score card of your team at this time in their soccer career.
